AmiSOX on aminet - it is a port of the unix SOX sound converter - it is a CLI program so if your more a GUI person there's a program that adds a GUI interface (SOXgui or something like that - I forget the name).
Speaking of converting to MIDI there is a program MOD2MIDI which will convert note and track data from a MOD file and save it as a .mid file. I haven't tried it as I heard it was buggy, but supposedly the latest version has the bugs pretty much worked out.
